  • Provide the necessary data for the identification of the medical history.
  • Present the ORIGINAL identification document of the user that will be attended at the time of registering at the reception of the institution.
  • Pay the value of the requested service, according to the permanent tariff manual within the clinic.
  • Provide accurate, complete and timely information related to your medical history, your current symptoms, problems and other matters related to your health or for reasons of general interest duly motivated, always with the respect due to the right of privacy and confidentiality.
  • Offer loyal and truthful personally identifiable information. Provide the data on the physical state that are necessary for your care process.
  • Use and enjoy the sanitary facilities and services, in a more responsible way, in accordance with the norms established therein, in order to guarantee their conservation and correct operation for use by all citizens.
  • Not to be the subject of an experimental investigation without it having been approved by the clinic and without its pain information and free consent.
  • To the guarantee of the right of access to the medical record by its owner, with the legally established limitations, as well as to obtain a copy of those elements of the same that he needs, without it being necessary to prove the reason for access.
  • Commit to informed consent.
  • To receive a service that guarantees the dignity, the preservation of the privacy of the person, the confidentiality of their information and the keeping of professional secrecy within the current ethical and legal framework of the health acts referred to.
  • To a kind and courteous treatment.
  • To be identified with your name.
  • To have your feelings respected and not violated.
  • To be informed about your state of health, the assistance plan and its evolution, taking into account your mood and according to your cultural and educational level.
  • To be informed verbally as a general rule to the user or his representative, in an understandable and adequate manner prior to the performance of an agreement, in order to express consent with sufficient knowledge and freedom.
  • To be attended by qualified personnel, with sufficient knowledge and skills, the user will have the right to be assigned a professional whose name he must know and who will be his valid interlocutor with the healthcare team.  In case of absence another team will assume responsibility.
  • That an integral, truthful and legible girl story is elaborated and preserved, as an instrument to coordinate, facilitate and evaluate your attention.
  • To the guarantee of the right of access to the medical record by its owner, with the legally established limitations, as well as obtaining a copy of those elements of the same that he needs, without it being necessary to prove the reason for access.
  • Receive clear and timely information about the risks and benefits of treatments.
